What is not offered

The following are services we cannot offer:

Those with an active workers compensation related claim cannot be treated at this office without direct authorization from the adjuster. We only work with Work Comp claims in Nevada and no other state. If you need a C-4 form completed, please go to either Concentra or UMC Quick Care.

This office is not contracted with most insurance companies, nor is it participating in-network with Medicare, Medicaid, or Culinary. If you are a Medicare beneficiary with a recent claim related injury, a referral to another facility is necessary. We are not with Blue Cross / Blue Shield.

We are not with culinary and are unable to make referrals for patients with culinary insurance.

If you are from the State of California, no provider in Nevada will accept Medi-Cal.

We no longer complete FMLA related paperwork at this office.

If you are a patient of the VA, you may be able to receive covered treatment. You will have to insist with your admin and doctor that they refer you directly to our office.

We do not provide disability related paperwork.

We are not performing DOT physicals under the National Registry of Certified Medical Examiners (NRCME) at this time.
